REQUIREMENTS (wording can be adjusted to meet your requirements)
1. Please read carefully and complete the sections of the form that apply for the role that you are applying for.
2. Submit a copy of any coaching certifications to the club labelling these with your name and the reference number provided at the end of this process.
3. For Competitive and All-Star Team Applicants (including coaches, assistants and managers) as well as those responsible for U18 House League teams, in order to be considered, you must:
 
    A. Fully complete and electronically sign and submit this form.
    B. Obtain/provide a Police Screening check. If you had a check done within the past 2 years, you may submit that form. If not then proceed as follows:
    • Complete a Police Screening Request Form and attach a photocopy of your driver's license, birth certificate or passport
    • Provide the Screening Request form to the club office. We will submit it to the police on your behalf. The results are returned directly to you, not to the club.
    • Obtain 3 written references on the form supplied by the club.
    C. Attend the scheduled interview. Bring the results (original form not a copy) of the Police Screening Request with you along with the completed personal reference forms.
4. Competitive and all-star team coaching applicants may be required to demonstrate their coaching capabilities (e.g. run a practice).

I agree to participate (or to allow this child to participate if signing on behalf of a minor) in the activities of the ABC Soccer Club. I understand and accept that there is a potential risk in training and participating in any sport; and, that the club tries to create a safe environment. I agree not to hold the ABC Soccer Club, its Directors or other club volunteers liable for any damages; loss; or injury sustained by me (or this child if signing on behalf of a minor) as a consequence of my/his/her participation in; or presence at: any programs, games or other activities of the ABC Soccer Club and I hereby release them from such claims.

I agree on behalf of myself/this minor child to abide by the rules, policies and decisions of the ABC Soccer Club and its governing bodies (YRSA, OSA, CSA, FIFA). The Club has established rules for participation. Proper conduct on and about the field must be followed. I understand that assignment to, and removal from a volunteer position with the ABC Soccer Club is at the discretion of the ABC Soccer Club Board of Directors and its delegated representatives.

I specifically grant permission to the ABC Soccer Club to use my/this minor child’s likeness, voice and words in television, radio, film, newsletters, magazines, and other media; and, in any form not heretofore described, for the purpose of advertising or communicating activities of the ABC Soccer Club.

I understand and agree that additional screening measures (beyond those outlined under "requirements" may be used, as deemed appropriate by the club, in order to validate my/this minor child's suitability for a volunteer position with the ABC Soccer Club.

If I/this minor child is selected for a volunteer position with the ABC Soccer Club, I agree on my behalf / on behalf of this minor child to attend Club clinics, information sessions and any other meetings or sessions that may be pertinent to my / this minor child's assigned role.
